ADVANCE DIRECTIVE
An advance directive speaks for you if you are unable to speak and helps to assure that your religious and personal beliefs will be respected. It is a useful document for an adult of any age to plan for future health care needs.
The North Queens Surgical Center accepts patients for services who are otherwise in good health.
- Patients are evaluated by physicians and nurses prior to the surgical experience or procedure.
- Although we do not expect any problems or complications in your scheduled procedure(s), an unforeseen complication could occur.
- At the time of the event, we would immediately institute emergency medical care, including but not limited to, life support and transfer you to the closest acute care hospital for follow-up care.
- Please bring a copy of your Advance Directive with you on the day of surgery
